I'm assuming this is to mark you day of birth, but is there a Liber or another book that could help me figure out how to determine said information, and is said information supposed to be written out, or drawn, so for instance, I would be "An gemini,..." but would I instead draw the symbol for gemini?"

I am also curious about this, there is software availble online which calculates the thelemic date, but is the form asking for your date of birth and zodiac degree, or is it asking for today's date to be listed in this format?
