Detriment is the sign opposite the sign a planet rules. Fall is the sign opposite the one in which it is exalted.

If a fall is the planet that opposes the exaltation of a given sign, then why is Mars the fall of signs wherein Jupiter is exalted...or Venus the fall of signs wherein Mercury is exalted? Should not Venus be the fall of signs in which Mars is exalted? or Mercury be the fall of signs in which Jupiter is exalted?

That's actually how the rulerships are setup. They are much more tied into planetary dynamics, and, in that pattern, Venus and Mars signs oppose each other as do Mercury and Jupiter signs.
The exaltations are a different plan that reflect these planetary dynamics. They do, however, reflect some Tree of Life dynamics much better (though I suspect this was not by design). Thus, Jupiter is exalted in Cancer and Mars in Capricorn (compare Chesed-Geburah). Venus is exalted in Pisces and Mercury in Virgo (compare Netzach and Hod). Down the middle of the hexagram would then be the Sun-Saturn axis (Aries-Libra), leaving only the Moon (the only planet exalted in a Hub constellation).**

How are the exaltations determined? Is there not an orderly system for them?

That's a book-length answer. (I could give it in a few sentences, but the few sentences would require pages of explanation.)
Until the mid 1940s, nobody knew their origins. Cyril Fagan discovered their origins and documented it in his book Zodiacs - Old & New.
